Graduating this Fall?

The Grad Tile will open on Mosaic on Wednesday, September 15 for students who completed their degree requirements during the Spring/Summer 2021 term.

Use the Grad Tile to:

• Update your name and address
  • Please ensure we have the address to which your diploma will be mailed
  • Only submit this form once
• Update how your name will appear on your diploma
  • Within the form you will be able to remove a middle name, correct capitalization and punctuation and request an initial only
• Apply for a Minor, Con-current Certificate and Mohawk certificate
  • If you need to update the content of any of the forms, you can do so by selecting ‘Update Graduation Form’
